This webinar will cover the basics of making proper assessments of residents prior to moving into your community.  Information will be presented on best practices, case studies and question and answer time.

Megan M. Graeser, DNP, GNP-BC    Megan completed her master's degree as a geriatric nurse practitioner and doctorate of nursing practice at University of Colorado Health Sciences Center, with a clinical research focus in medication management in the elderly population.  She was first introduced to house call medicine during her clinical rotations in her nurse practitioner program and found it to be her ideal practice setting.  Megan joined Physician Housecalls in 2007 and finds it a privilege to care for patients in their homes.  She enjoys exposing nurse practitioner students to house call medicine and geriatrics through her time as clinical preceptor.   Megan lives in Southwest Littleton with her husband and their two young sons.  She enjoys hiking, snowshoeing, and just spending time outside in beautiful Colorado with her family.
